Home health staff provide care in your own home, or in a temporary care facility. Our caregivers offer support to manage illness and recover from injury or surgery. The team includes nurses, doctors, social workers, physical therapists, occupational therapists, registered dietitians and home health aides.
Hospice provides expert care and support to patients and families facing serious advanced illnesses. Our care is centered on the patient’s goals and personal choices; they tell us what’s important to them and we provide care tailored to their wishes. Hospice care focuses on pain relief and symptom control so people can live more comfortably and enjoy an enhanced quality of life.
Private Duty care is available seven days a week to provide in-home assistance. Private Duty care can be arranged for a minimum of 3 hours and up to 24 hours per day, and includes Certified Nursing Assistants / Certified Home Health Aides and Licensed Vocational Nurses.
Palliative care helps alleviate symptoms, manage pain and stress, and improve the quality of life associated with a serious illness whatever the diagnosis. Palliative care is appropriate for any age or stage of illness and is often provided alongside curative treatment.
Support for you and your family does not end with the death of your loved one. Our families receive 13 months of grief support to help them understand and navigate through the grief process. During those 13 months, you will receive follow-up phone calls and a quarterly grief newsletter. We also provide bereavement counseling along with several grief support groups.
